KINDERGARTEN REGISTRATION

2026-2027

London City Schools is thrilled to prepare for the arrival of the Red Raider Class of 2039.  If you have a child who will be 5 years old on or before August 18, 2026, they are eligible to enroll in kindergarten for the 2026-2027 school year.  Our registration week will be April 27 – May 1. Enrollment is a two-step process.  Your student will not be enrolled until both steps have been completed.

 

Step 1

Please visit Final Forms at https://london-oh.finalforms.com/  Click on Parent *New Account* to register your child and start the enrollment process.  You will create only one parent account to add all of your children.  These forms must be completed prior to your in-person registration meeting and in order for your student to attend school.

 

Step 2

Once you have all of your forms completed you will receive an email with a link to schedule an appointment during the week of April 27 – May 1.  Your child does NOT need to be present during the registration process, screenings will be completed at a later date.  If your child is already enrolled at our London Preschool then you do not need to enroll them again. 

Required Documents to bring with you to the appointment

1) Parent ID

2) Student Birth Certificate

3) Custody paperwork (if applicable) 

4) Proof of Residency= 2 forms required, lease, utility bills or mortgage statement.  (If you are doing a Resident Affidavit then that person will need to come with you to the enrollment appointment)

5) Immunizations 

6) Physical (KG students only)

IMMUNIZATIONS and PHYSICAL 

The State Department of Health and London City Schools require the following:

MMR – Must be given to the child on or after first birthday (2 doses)

Dtap – 5 doses (Basic series and a booster after age 4)

Polio Vaccine – 4 doses (basic series and a booster after age 4)

Hepatitis B – 3 doses

Varicella (Chicken Pox) – 2 doses after first birthday

 

A physical examination will be required prior to school starting in August. Physical examinations done within 1 year prior to school starting will be accepted.  If your child has had a physical examination done in August 2025 to present, please bring it with you to registration.  Please note, if a student’s immunizations are not up to date and a current physical has not been turned in, your child will not be permitted to attend school after September 4, 2026.  If you are unable to pay for your vaccinations, please call the Madison County Health Department for assistance.  If your doctor’s office requests a physical form, please click HERE.  If you have any questions, please feel free to call us at (740) 852-5700 ext 2117.  We look forward to meeting you.
