KINDERGARTEN REGISTRATION
2021-2022
If you missed our Kindergarten registration week don't worry you can still register this summer. Starting June 7th you can come into Central Office at 380 Elm St, Monday-Thursday between the hours of 9am - 11am and 1:30pm - 3pm.
Students must be 5 years of age by August 1, 2021 to be eligible. Your child does NOT need to be present during the registration process. Screenings will be completed at a later date. If your student attends Fairhaven Preschool and will be attending London you do NOT need to register.
Required Documents are as follows:
1. Completed Enrollment Packet, click here for packet
2. Parent Photo ID
3. Official Birth Certificate - Not the certificate from the hospital with footprints.
4. Proof of Residence - Lease agreement, auditors page showing ownership of home, utility bill. If you are living with someone contact our office
5. Custody Documents - If applicable
6. Immunizations
7. Current Physical
IMMUNIZATIONS AND PHYSICAL
The State Department of Health and London City Schools require the following:
MMR – Must be given to the child on or after first birthday (2 doses)
Dtap – 5 doses (Basic series and a booster after age 4)
Polio Vaccine – 4 doses (Basic series and a booster after age 4)
Hepatitis B – 3 doses
Varicella (Chicken Pox) – 2 doses after first birthday
A physical examination will be required prior to school starting in August. Physical examinations done within 1 year prior to school starting will be accepted. If your child has had a physical examination done in August 2020 to present, please bring it with you to registration. Please note, if a student’s immunizations are not up to date and a current physical has not been turned in, your child will not be permitted to attend school after September 2, 2021. If you are unable to pay for your vaccinations, please call the Madison County Health Department for assistance. If your doctor’s office requests a physical form, please click here.
